I understand that IBS, Inc. follows an "employment at will" policy, in that I or IBS, Inc. may terminate my employment at any time, or for any reason consistent with applicable state or federal law. This "employment at will" policy cannot be changed verbally or in writing, unless the change is specifically authorized in writing by an officer of IBS, Inc. I understand that this application is not a contract of employment. I understand that federal law prohibits the employment of unauthorized aliens. All persons hired must submit satisfactory proof of employment authorization and identity. Failure to submit such proof will result in denial of employment.

I understand this application will be active for a period of one year. After that time, if I wish to be considered for employment, I must submit a new application.

In the event that the applicant terminates employment within the first 60 days, a deduction from their final paycheck (not to exceed $175) may occur in order to compensate for Ohio security license fees as well as fees associated with performing the initial background check.

In connection with my employment (including contract for services) with Industrial Business Services, Inc., hereinaftercalled "Employer", do hereby authorize Employer to perform background inquiries on myself, including but not limited to: consumer credit, criminal arrests/convictions, motor vehicle reports, academic records, legal history and other reports. These reports will include information as to my character, skills and work habits, performance and experience along with reasons for termination of past employment from previous employers. I further authorize any person, form, corporation or other entity to furnish said information. I understand that I must accept this document in order for my application for employment to be considered further.

I further release, discharge and hold blameless Employer and any party providing information to Employer as a result of this authorization, from any liability, claims or causes of action which I or my heirs, executors or assigns may have as a result of the delivery, disclosure, non-disclosure or omission of any information in connection herewith.

I further understand and agree that the information for this background inquiry will be requested (interviews and/or written requests) from various federal and State agencies, former employers (except current employer, if so noted), academic institutions, credit reporting agencies, financial sources, governmental agencies as well as any other entity that maintains records concerning my past activities relating to my driving record, credit, criminal, civil and other experiences.

My acceptance below indicates that all statement and representations made by me to Employer are true, and I understand that any misrepresentation or omission of significant or substantive information will be sufficient cause for termination. I also understand that, while under consideration for employment, or once employed, I must notify Employer of any conviction, other than a parking violation, within three (3) working days.

Should I become employed by IBS, Inc., I hereby agree to submit to periodic checks of my background by Employer, at Employer's sole discretion. These checks may include inquiries of the same magnitude and extent as authorized in the pre-employment section of the authorization. The timing of these periodic checks shall be at the sole discretion of the Employer. I further agree that if I attempt to withdraw this authorization at any time during my employment with Employer, said withdrawal shall be deemed justification for immediately terminating my employment.
